Job summary

The Brookdale Hospital Medical Center is seeking a Case Manager with a strong clinical background to assess medical necessity, coordinate care, and advocate for timely, safe discharge planning. This role works with physicians, residents, social workers, and nursing staff to optimize patient outcomes.

Applicants should have a BSN, NY RN licensure, BLS, and PRI certification, with 3+ years of clinical experience in acute care or related settings. CCM/PALS/ACLS are preferred.

Qualifications

• Bachelor's degree in Nursing (BSN) is required.
• Minimum 3 years of clinical experience across acute care, medical-surgical, ICU, ED, home care or similar.
• Current New York State RN licensure and BLS required.
• PRI certification required; PALS/CCM/ACLS preferred.
• Knowledge of utilization review, discharge planning, and CMS/MCG guidelines.

Responsibilities

• Perform face-to-face patient assessments to identify barriers to self-care and arrange community referrals.
• Review daily EHR for medical necessity and coordinate with interdisciplinary teams.
• Collaborate with social workers to address social determinants of health affecting outcomes.
• Communicate admission guidelines with Attending physicians and ensure timely documentation in EPIC.
• Document care plans, PRI reviews, and discharge planning activities for accuracy and compliance.
