MetroWest Medical Center in Framingham, MA is seeking an experienced Registered Nurse Case Manager to coordinate care across the continuum, promote timely discharge, and prevent readmissions. You will collaborate with physicians and care teams, assess patient transition needs, and ensure compliance with regulatory standards while supporting patient-centered care and resource utilization.

Ideal candidates will have a solid clinical background in acute hospital settings, strong documentation

Formación

• RN with active license in appropriate state.
• Minimum 2 years of acute hospital patient care experience.
• Active RN license for state(s) covered.

Responsabilidades

• Facilitate care across the continuum and coordinate resources.
• Promote timely throughput, safe discharge, and prevent readmissions.
• Document thoroughly in Case Management system and collaborate with care teams.
• Identify and refer complex patients to Social Work Services.
• Ensure compliance with regulatory requirements and standards.
