Qualifications

• BSN or higher from an accredited program
• Active RN license with 1+ years experience in case management
• Exceptional interpersonal and communication skills, written and verbal
• Proficiency in electronic health records (EHR) software

Responsibilities

• Conduct comprehensive assessments of patients and psychosocial needs
• Collaborate with healthcare providers, patients and families to develop care plans
• Monitor progress and adjust care plans as needed
• Serve as main point of contact between patient and providers for seamless coordination
• Maintain up-to-date records and ensure confidentiality and compliance
