Mass General Brigham in Boston seeks an experienced RN Case Manager to join the Case Management department. The role focuses on coordinating patient care across the continuum, improving the patient experience, and supporting high-quality outcomes in an on-site hospital setting.

The Case Manager collaborates with physicians, nurses, and care teams to assess needs, plan care, and optimize reimbursement while maintaining clinical excellence and compassionate patient support.

Qualifications

• RN Case Manager experience required.
• Must be a licensed RN in the state of Massachusetts.
• Minimum of 2 years Med/Surg experience required.
• Case Manager certificate preferred.
• ADN required, but BSN strongly preferred.
• Acute hospital experience preferred.

Responsibilities

• Collaborates with the health care team to enhance patient care delivery.
• Evaluates, plans, and coordinates patient care across the continuum.
• Assesses patient needs and advocates for timely, efficient care.
• Monitors patient progress and ensures alignment with institutional standards.
• Supports cost reduction and appropriate reimbursement through effective care management.
