Job summary

Union Health in Terre Haute, IN is seeking an experienced RN for Case Management - Behavioral Health 4W. The role is full-time, day shift, Monday through Friday, 7:00am to 3:30pm, with responsibilities across care coordination, discharge planning, and family education.

The ideal candidate will have an ASN or BSN, Indiana RN license, and RN experience; BLS certification required. Join a Magnet-designated team focused on quality, patient outcomes, and professional growth.

Qualifications

• Graduate from an accredited ASN or BSN program.
• Must have Indiana RN license.
• Previous RN experience preferred.

Responsibilities

• Assesses the patient and family for continuing care needs, including education and financial needs.
• Serves as resource to the unit and works in staffing as necessary.
• Participates/leads recovery planning.
• Meets with family members as necessary for visitation.
• Reviews medical records and interviews patient and/or family as appropriate; assesses and investigates patient's needs and acts accordingly.
• Helps to determine and coordinate plans of care, including physical, financial, and psychosocial needs.
• Coordinates with physicians, nursing, and Utilization Review to confer if interventions need to be modified or adjusted.
• Conducts auditing of charts and creates action plans to meet regulatory and quality of care standards.
