Job summary

Johnson Memorial Health in Franklin, IN is seeking a Behavioral Health RN Case Manager to assess patient records, apply Interqual criteria, and coordinate care across the continuum.

Requires BSN and Indiana RN license; 1 year in behavioral health and case review in acute care preferred. Knowledge of managed care, Medicare/Medicaid guidelines, and discharge planning is a plus. Monday–Friday, 8:00 a.m.–4:00 p.m.

Qualifications

• BSN required and Indiana RN license active.
• One year behavioral health experience required.
• One year case review experience in acute care preferred.
• Clinical pathway experience and certification in Case Management preferred.
• Familiar with managed care, Medicare and Medicaid guidelines; precertification and reviews.

Responsibilities

• Assess and analyze patient records for severity of illness and service needs.
• Collaborate with the healthcare team to ensure appropriate care placement.
• Complete reviews using Interqual criteria.
• Coordinate insurance certifications and discharge planning with required timelines.
• Document discharge plans to ensure regulatory compliance.
• Communicate with care team to resolve barriers and ensure smooth transitions.
