Job summary

St. Croix Hospice is seeking a compassionate Registered Nurse Case Manager to lead in-home patient care. You will assess needs, develop personalized plans, and collaborate with a multidisciplinary team to ensure symptom control and dignified end-of-life support.

The role requires an RN and 1+ year of acute care experience, with hospice/home health preferred; on-call duties accompany a full-time daytime schedule in Ankeny, IA.

Qualifications

• RN license required (associates/bachelors/masters in nursing)
• 1+ year of acute care experience; hospice/home health preferred
• Strong patient-focused communication and teaching skills

Responsibilities

• Conduct initial and ongoing assessments to determine hospice needs
• Develop and revise individualized care plans with patient and family
• Administer medications and treatments per physician orders
• Educate patients and caregivers on care techniques, meds, diet, and symptom management
• Coordinate with the care team for seamless transitions and on-call coverage
