Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to deliver holistic, in-home care to patients and their families, guiding end-of-life planning with compassion and clinical excellence.

You will complete initial assessments, develop care plans with families, administer medications, educate caregivers, and rotate on-call duties to ensure seamless transitions.

Qualifications

• RN degree from an accredited program (ADN/BSN/MSN).
• 1+ year of acute care experience required.
• Hospice/home health experience preferred.
• Must have reliable transportation and a valid driver’s license.

Responsibilities

• Assess patients and families to determine hospice needs.
• Develop individualized care plans with patient and family.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques and symptom management.
• Rotate on-call duties to ensure seamless care transitions.
