Job summary

St. Croix Hospice seeks a Registered Nurse Case Manager to deliver individualized, holistic in-home care. You will lead end-of-life conversations, collaborate with a care team, and ensure compassionate symptom management with clinical excellence.

You will complete assessments, develop care plans with patients and families, administer medications, and educate caregivers while maintaining autonomy in practice and ethical standards.

Qualifications

• RN must hold an active nursing license in good standing.
• Minimum 1 year of recent acute care experience required.
• Hospice/home health experience is preferred.

Responsibilities

• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patient and family and update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, diet, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
