Full time, salaried, exempt position
Work Schedule: Monday-Friday, 8am-5pm
Service area: Alamance County

This position demonstrates competency in delivery of quality patient care to patients ranging in age from infants to geriatrics. The RN Case Manager is responsible for developing and implementing a plan of care in accordance with the patient’s expressed wishes and delegates care and interventions in full compliance with the Nurse Practice Act and organizational policy, procedures and standards of care. In addition, this position supports hospice growth and access to care by serving as a resource to referral sources, completing hospice intake and admissions as needed, and coordinating patient transitions through education, care coordination, and continuity from hospital or community settings into hospice services.

Essential Duties
  • Serves as clinical resource for assigned hospital(s), facilities, and referral partners.
  • Provides education to patients, families, and referral sources regarding hospice eligibility, philosophy, and services.
  • Supports organizational growth in assigned service areas through relationship‑building and service visibility.
  • Plans, provides, and documents safe, compassionate patient care in accordance with the established plan of care, physician orders, the Nurse Practice Act, Hospice Standards of Practice, and organizational policies.
  • Assesses physical, environmental and safety needs of patient/family by completing the initial comprehensive assessment. Reviews all medications the patient is taking, reconciles list for medical record, and checks for drug/drug interactions. Collaborates with a medical provider for additional medication orders as needed.
  • Provides initial and ongoing education to patients and caregivers on safe home care and program requirements, ensuring understanding and adherence to the current plan of care.
  • Develops and coordinates patient care plans in collaboration with patients, families, and the interdisciplinary team to achieve desired outcomes and care goals, while maintaining professional collaboration with physicians and healthcare providers and communicating timely, pertinent clinical information to appropriate team members.
  • Provides accurate and timely documentation of nursing assessment, interventions, and updates to Plan of Care according to the nursing standards of this organization.
  • Delegates tasks and supervises the activities of nurse aides and LPNs and documents supervision according to organizational policy.
  • Serves as an active member of the interdisciplinary group (IDG) and attends IDG meetings regularly.
