Job summary

Temple Hospital is seeking a Registered Nurse Case Manager for 40 hours per week on the Day Shift to coordinate care for patients through the acute episode. You will collaborate with the patient/family, physicians and the interdisciplinary team to ensure efficient, cost-effective progression of care.

You will monitor care levels, initiate evidence-based protocols, and propose transition plans within 48 hours of admission, acting as an advocate for patients and families throughout the acute

Qualifications

• BSN is required; alternative candidate with relevant education and experience may be considered if committed to obtaining BSN within 2 years from hire date.
• 1 year of experience in acute care Case Management or Utilization Management in acute care, facility, or insurance setting is required.
• Licensed RN in PA or Multi-State CompactRN license required; Certified Case Manager preferred.

Responsibilities

• Collaborate with patient/family, physicians and interdisciplinary team to guide patient through acute care.
• Monitor level of care and initiate evidence-based protocols and order sets.
• Develop and propose an appropriate transition plan within 48 hours of admission.
• Advocate for patient and family throughout the acute episode of care.
