Job summary

Temple Health – Temple University Health System is seeking an experienced Case Manager to collaborate with patients, families, physicians, and the interdisciplinary team to guide care through the acute episode efficiently and cost-effectively. The role emphasizes ensuring appropriate transitions within 48 hours of admission and advocating for patients and families throughout the episode.

The position requires a BSN (or equivalent with commitment to obtain BSN within 2 years), and at least 1 year

Qualifications

• BSN required or combination of relevant education and experience may be considered in lieu of degree with commitment to obtain BSN within 2 years from date of hire

Responsibilities

• In collaboration with the patient/family, physicians, and the interdisciplinary team, the Case Manager facilitates the patient's progress through the acute episode of care in an efficient and cost effective manner.
• Monitors the level of care, initiating evidence based protocols and order sets according to the patients' admission diagnosis, influencing the progression of care, facilitating the patients' navigation through the acute care episode, and proposes an appropriate transition plan within 48 hours of admission.
• The Case Manager serves as an advocate for the patient and family throughout the entire acute episode of care.

Education
