Temple Health's RN Case Manager at Temple Hospital collaborates with patients, families, physicians, and the interdisciplinary team to move the patient through an acute episode of care efficiently and cost-effectively.

The role monitors level of care, applies evidence-based protocols, and develops a transition plan within 48 hours of admission while serving as an advocate throughout the episode.

Qualifications

• BSN required or equivalent with commitment to obtain BSN within 2 years from date of hire.
• 1 year experience in acute care Case Management or Utilization Management.
• General experience in case management; knowledge of InterQual criteria preferred.
• PA RN license required or Multi-State Compact RN license; CCM preferred

Responsabilités

• Collaborate with patient/family, physicians, and interdisciplinary team to facilitate progress through acute episode of care.
• Monitor level of care; initiate evidence-based protocols and order sets.
• Influence progression of care and develop transition plan within 48 hours of admission.
• Advocate for patient and family throughout the acute episode of care
