Job summary

St. Croix Hospice is seeking a Registered Nurse Case Manager to provide 1:1, whole-person care to patients and families in their homes. You will partner with an interdisciplinary team to coordinate personalized care plans and support families through difficult decisions with empathy and respect.

This full-time role requires an RN with 1+ years of acute care experience; hospice/home health experience is preferred. On-call duties and reliable transportation are part of the position.

Qualifications

• RN licensure required (state of WI preferred) and current CPR/BLS.
• 1+ years of recent acute care experience; hospice/home health preferred.
• Complete initial and ongoing comprehensive assessments to determine hospice needs.
• Develop individualized care plans with patients and families; update as needs evolve.
• Administer medications and treatments as prescribed.
• Educate patients and caregivers on care techniques, medications, and symptom management.
• Rotate on-call duties and ensure seamless care transitions.
