Job summary

Elara Caring is hiring an RN Case Manager to deliver home health services across Youngstown, Warren, Cortland, Poland, Boardman and surrounding communities. You will assess patients, develop personalized care plans, and provide skilled nursing in patients' homes.

Travel up to 50% is required. A current RN license and 1+ year of clinical nursing experience are preferred; a $5,000 sign-on bonus is offered to eligible candidates.
