Job summary

Cynet HealthStaff is seeking a dedicated Registered Nurse to serve as a Home Hospice Case Manager for a 13-week assignment in Williamsburg, Virginia. The role requires a valid RN license, BLS certification, and hands-on experience in home hospice case management.

You will manage a census of 35–45 patients, document in EPIC, and collaborate with an interdisciplinary care team to deliver compassionate home-based care. Day shifts, 8 hours per shift, with flexibility to float as needed.
