About the Role:

The RN Case Manager - Home Hospice plays a critical role in delivering compassionate, patient-centred care to individuals facing life-limiting illnesses in their homes. This position involves comprehensive assessment, care planning, and coordination of hospice services to ensure patients' comfort and dignity during end-of-life care. The RN Case Manager collaborates closely with interdisciplinary teams, patients, and families to address physical, emotional, and psychosocial needs. They serve as a primary clinical resource, advocating for patient preferences while managing symptom control and quality of life. Ultimately, this role ensures seamless hospice care delivery that aligns with regulatory standards and ethical practices across diverse home settings.

Minimum Qualifications:
  • Current and valid Registered Nurse (RN) license in the United States.
  • Minimum of two years of clinical nursing experience, preferably in hospice, palliative care, or related fields. New graduates may be considered.
  • Ability to work independently in a home care setting with excellent organizational and time management skills.
  • Proficiency in electronic medical records (EMR) and documentation practices.
