Job summary

Moments Hospice is looking for an experienced RN Case Manager to lead clinical care for patients and families in a hospice setting. You will assess needs, manage symptoms, and coordinate multidisciplinary care with compassion, guiding families through end-of-life decisions.

Qualifications

• Active RN license in the state you work in.
• Experience in hospice or palliative care preferred.
• Strong communication, organization, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Valid driver’s license and reliable vehicle.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

RN Case Manager – Hospice

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
