Job summary

Addus HomeCare’s Alamo Hospice is hiring an RN Case Manager to join a growing home-based team in the Waco area. This field-based role manages patient and family caseloads, coordinates care, and provides therapeutic nursing in patients' homes.

The position covers Waco, TX and surrounding counties, with a Monday–Friday schedule and on-call rotation. Comprehensive benefits, mileage reimbursement, tuition assistance, and career growth opportunities are offered.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Must possess current CPR and First Aid Certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Manage a patient and family caseload across the nursing process (assessment, planning, implementation, evaluation).
• Collect data on patient’s physical, psychological, social, and spiritual factors affecting needs.
• Coordinate care with attending physicians, interdisciplinary team, and other agencies.
• Conduct regular patient visits to reassess needs and update the plan of care.
• Provide support to families and perform clinical pronouncement of death when needed.
• Maintain accurate patient records, charts, and care plans.

Alamo Hospice is seeking an RN Case Manager to join their growing team in Waco! This field-based position is responsible for managing a caseload of patients, providing therapeutic nursing, coordinating care, and supporting patients and their families in the comfort of their homes.

Territory: Waco, TX and the surrounding counties.

Schedule: Monday-Friday 8am-5pm. On-Call-1-2 weeknights a week and 1 weekend every 4-5 weeks.

What You'll Do

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines
