Job summary

Ambercare Hospice, part of the Addus HomeCare family, is seeking a compassionate Registered Nurse Case Manager in Silver City, NM. You will provide expert clinical care to patients and families, supporting comfort, dignity, and quality of life in a home health/hospice setting.

The role offers a Monday–Friday daytime schedule with autonomy to manage patient visits, competitive pay with mileage, and growth opportunities within a nationwide hospice network.

Qualifications

• Graduate from an accredited registered nursing program.
• Licensed as a registered nurse in the state of practice.
• Current CPR and First Aid certification.
• Knowledge of quality assessment and assurance procedures.
• Valid driver’s license and proof of insurance is required.

Responsibilities

• Assume primary responsibility for patient and family caseload, including assessment, eligibility, admission processing, planning, implementation, and evaluation of the nursing process.
• Obtain data and assess information from patient and family factors that may impact care and coordinate with interdisciplinary team.
• Initiate communication with attending physician, interdisciplinary team, and other agencies to coordinate optimal care.
• Provide regular visits to re-assess the patient’s needs and update the plan of care.
• Perform clinical pronouncement of death and provide support to the family.
• Maintain accurate patient records, charts, progress notes, and care plans.
• Supervise and document delivery of care by Home Health Aide as required by guidelines.

What You’ll Do:

• Assume primary responsibility for patient and family caseload, including the assessment, determining eligibility, admission processing, planning, implementation, and evaluation phases of the nursing process
• Obtain data and assess necessary information from patient physical, psychological, social, and spiritual factors that may impact the patient and family's needs and initiate intervention by other members of the interdisciplinary team
• Initiate communication with the attending physician, interdisciplinary team, and other agencies for the purpose of coordinating optimal care
• Provide regular visits to re-assess the patient's needs and update the plan of care
• Perform clinical pronouncement of death and provide support to the family
• Maintain accurate patient records, charts, progress notes, reports and prepare care plans timely
• Supervise and document delivery of care by the Home Health Aide as required by Federal, State, and Company guidelines

Qualifications:

• Graduate from an accredited registered nursing program
• Licensed as a registered nurse in the state of practice
• Must possess current CPR, First Aid Certification
• Must be knowledgeable of quality assessment and assurance procedures
• Valid driver’s license and proof of insurance is required
