Job summary

Crossroads-Home-Health-and-Hospice is seeking a Registered Nurse to join our Home Health team in Vallejo, CA. The RN will provide skilled nursing care to patients in their homes, develop individualized care plans, administer medications, and educate families to promote independence and quality of life.

Ideal candidates will have CA RN licensure, at least 1 year of home health experience, CPR, and strong communication skills. The role requires both independent work and teamwork within a care team.

Qualifications

• Active and unrestricted RN license in California.
• Minimum of 1 year of recent Home Health or related field experience.
• Current CPR certification.

Responsibilities
