Job summary

Moments Hospice is seeking an RN Case Manager to lead clinical care for a caseload of patients at home or in community settings. You will assess needs, develop and adjust plans of care, administer medications as prescribed, and support families through end-of-life care.

This full-time, Monday–Friday position offers a pay range of $75,000 to $90,000 per year, a fleet car program, comprehensive health benefits, and 401(k) with company match.

Qualifications

• Active RN license in the state you work in.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.
• Compassionate, patient- and family-centered approach.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across your caseload and serve as the primary liaison among patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.
