Job summary

Moments Hospice is seeking an RN Case Manager to serve as the clinical lead for patients and families in a field-based role. You will coordinate care, manage symptoms, and provide guidance during end-of-life care, with a supportive team and reasonable caseloads.

Qualifications

• Active RN license in the state of practice.
• Proficiency with EMR systems.
• Strong organizational and communication skills.
• Ability to work independently with high standards.

• Valid driver's license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing assessments of patients' physical, emotional, and psychosocial needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and end-of-life expectations.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM
