Job summary

Moments Hospice seeks an experienced RN Case Manager to lead clinical care for patients and families in a field-based role. You will assess needs, coordinate plans, manage symptoms, and support families through end-of-life care.

Qualifications

• Active RN license in the state of work.
• Proficiency with electronic medical records (EMR).
• Strong organizational, communication, and critical-thinking skills.

• Valid driver’s license and reliable vehicle with current insurance.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' needs.
• Develop, implement, and update individualized care plans with patients and families.
• Provide skilled nursing care including pain and symptom management.
• Educate patients and caregivers on disease progression and care expectations.
• Coordinate care across caseload and liaise with patients, families, physicians, and the care team.
• Participate in interdisciplinary group meetings to ensure holistic care.

RN Case Manager – Hospice
Pay:$75,000 – $90,000 per year
Job Type: Full-time | Field-based
Schedule: Monday–Friday, 8:30 AM – 5:00 PM

Responsibilities:

• Complete initial and ongoing comprehensive assessments of patients' physical, emotional, and psychosocial needs
• Develop, implement, and update individualized plan of care in partnership with patients and families
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care

Qualifications:

• Active Registered Nurse (RN) license in the state you work in
• Proficiency with electronic medical records (EMR)
• Strong organizational, communication, and critical-thinking skills
• Ability to work independently while maintaining high standards of care
• A compassionate, patient- and family-centered approach
• Valid driver's license and reliable vehicle with current insurance
