Job summary

LMH Health in Kansas is seeking an RN Case Manager to collaboratively assess, plan, implement, coordinate, monitor and evaluate options and services for optimal, cost-effective patient outcomes. You will engage with patients, families, physicians and the care team to promote high-quality care.

The role requires an RN license in Kansas, 1 year in case management/discharge planning, 3 years in medical/surgical settings, and familiarity with InterQual to support appropriate care decisions.

Qualifications

• Registered Nurse licensed in Kansas.
• One year hospital case management and/or discharge planning experience.
• Three years medical/surgical experience in a clinical setting.
• Experience utilizing InterQual.

Responsibilities

• Gather data from chart review and patient interview.
• Identify barriers and set realistic goals with potential alternatives.
• Develop discharge plans with patients, family, physician, and care team.
• Modify plans as treatment progresses and monitor outcomes.
• Promote clear communication among care teams, patients, and families.
• Ensure appropriate use of resources and progression of care.
• Coordinate with third party payors to maximize benefits.
• Educate patients and families on resources and regulations.
• Advocate for patient care and timely progress while maintaining attendance.

Job Description

JOB SUMMARY The RN Case Manager is a collaborative process which assess, plans, implements, coordinates, monitors and evaluates options and services to meet the patient’s needs through communication and available resources to promote quality cost-effective outcomes.

ESSENTIAL JOB RESPONSIBILITIES

• Gather all relevant data from chart review and patient interview.
• Objectively identify barriers, clarify/determine realistic goals and seek potential alternatives.
• Work with patient/family/physician/healthcare tam to develop a treatment/discharge plan to promote positive patient outcomes and cost effective care.
• Initiate and implement plan modifications secondary to treatment response through continuous monitoring and communication with the healthcare team.
• Actively promote communication between healthcare team members, patient/family, and physicians.
• Monitor and promote appropriate use of resources, patient status and level of care.
• Communicate with third party payors to ensure payment and maximize benefits.
• Support and educate patient/family, physician, and healthcare team on available resources and regulations.
• Represent the best interest of the patient and hospital by advocating for necessary funding, treatment alternatives, timelines and coordination of health services and re-evaluation of progress and goals.
• Regular and reliable attendance is an essential function of this position.

JOB QUALIFICATIONS

• Registered Nurse
• Current Nursing License in the State of Kansas
• One year Hospital Case Management and/or Managed Care and Discharge Planning experience
• Preferred: Bachelor of Science in Nursing Case Management Certification
• Three years Medical/Surgical experience in a clinical setting
• Experience utilizing InterQual
