LMH Health is seeking an RN Case Manager to coordinate patient care from admission to discharge in Kansas. The role emphasizes collaboration with the health care team and patient/family education to promote quality, cost-effective outcomes.

Essential responsibilities include data gathering, barrier assessment, care planning, plan modification, and active communication with the care team and payors. A current KS nursing license and case management experience are required.

Qualifications

• RN licensure required
• Current Nursing License in the State of Kansas
• 1 year hospital case management and/or managed care and discharge planning experience

Responsabilités

• Gather all relevant data from chart review and patient interview
• Identify barriers and set realistic goals with patients and care team
• Develop a treatment/discharge plan with patient/family/physician to promote outcomes and cost-effective care
• Implement plan modifications based on treatment response and ongoing monitoring
• Facilitate communication among care team, patient/family, and physicians
• Monitor resource use, patient status and level of care
• Coordinate with third-party payors to ensure payment and maximize benefits
• Educate patient/family, physician and team on resources and regulations
• Advocate for funding, treatment alternatives, timelines and coordination of health services

Job Description

JOB SUMMARY

The RN Case Manager is a collaborative process which assess, plans, implements, coordinates, monitors and evaluates options and services to meet the patient’s needs through communication and available resources to promote quality cost-effective outcomes.

ESSENTIAL JOB RESPONSIBILITIES

• Gather all relevant data from chart review and patient interview.
• Objectively identify barriers, clarify/determine realistic goals and seek potential alternatives.
• Work with patient/family/physician/healthcare tam to develop a treatment/discharge plan to promote positive patient outcomes and cost effective care.
• Initiate and implement plan modifications secondary to treatment response through continuous monitoring and communication with the healthcare team.
• Actively promote communication between healthcare team members, patient/family, and physicians.
• Monitor and promote appropriate use of resources, patient status and level of care.
• Communicate with third party payors to ensure payment and maximize benefits.
• Support and educate patient/family, physician, and healthcare team on available resources and regulations.
• Represent the best interest of the patient and hospital by advocating for necessary funding, treatment alternatives, timelines and coordination of health services and re-evaluation of progress and goals.
• Regular and reliable attendance is an essential function of this position

JOB QUALIFICATIONS

Required:

• Registered Nurse
• Current Nursing License in the State of Kansas
• One year Hospital Case Management and/or Managed Care and Discharge Planning experience

Preferred:

• Bachelor of Science in Nursing
• Case Management Certification
• Three years Medical/Surgical experience in a clinical setting
• Experience utilizing InterQual
