Job summary

AMN Healthcare is seeking a RN Case Manager for Sitka, AK on a 13-week assignment. You will collaborate with providers and the interdisciplinary team to coordinate care and facilitate referrals and follow-up services.

You will document assessments, interventions, and outcomes in Meditech, and advocate for patients navigating the healthcare system. Qualified candidates hold an unrestricted AK RN license, BLS, an Associate’s degree in Nursing, and at least 1 year of case management experience

Qualifications

• Unrestricted AK RN license.
• BLS certification.
• Associate’s degree in Nursing.
• 1 year of case management experience within the last 3 years.

POSITION SUMMARY – RN Case Manager

POSITION DUTIES

Collaborates with providers and interdisciplinary team members to coordinate care. Facilitates referrals, follow-up care, and continuity of services. Documents assessments, interventions, and outcomes accurately and timely in the electronic health record. Advocates for patient care and assists patients in navigating the healthcare system.

Minimum Required Qualifications

• Unrestricted AK RN license
• BLS
• Associate’s degree in Nursing from an accredited program
• 1 year of case management experience within the last 3 years
