Aegis Healthcare in Scottsdale, AZ is seeking an RN Case Manager to deliver compassionate home health and hospice care. You will assess patients, collaborate with families, and develop individualized care plans that support safe, effective in-home treatment.

This full-time role offers flexible scheduling, a supportive culture, generous benefits, and opportunities for growth. Must have AZ RN license, Home Health/Hospice experience preferred, CPR, TB test, and a valid AZ Fingerprint Clearance Card.

Qualifications

• Active Registered Nurse (RN) license in Arizona.
• Experience in Home Health or Hospice preferred.
• CPR certification required.
• Valid driver’s license and reliable transportation.
• AZ Fingerprint Clearance Card (assistance available).

• Conduct comprehensive physical, social, and psychosocial assessments.
• Collaborate with patients, families, and care teams to create individualized care plans.
• Manage care delivery and maintain accurate patient records.

Position Details

STATUS: Full-Time (mon-friday 8am-5pm w/ one 24 hour oncall shift per month)

COMPENSATION: $40-$45 /hour

LOCATION: Scottsdale, AZ

Your Role As RN Case Manager

As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. Your role will include:

• Conducting comprehensive physical, social, and psychological assessments.
• Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Managing care delivery and maintaining meticulous patient records.
• Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.

Qualifications We're Looking For

• Active Registered Nurse (RN) license in Arizona (or state of practice).
• Previous Home Health or Hospice experience (preferred).
• Effective verbal, written, and time management skills.
• Valid driver's license, reliable transportation, and proof of current auto insurance.
• Ability to perform patient transfers with a full range of movement.
• Current CPR certification and negative TB test (we can assist).
• Valid AZ Fingerprint Clearance Card (we can help obtain it).
