Job summary

Aegis Healthcare is seeking a dedicated RN Case Manager to lead Home Health & Hospice nursing for our Scottsdale team. You will conduct comprehensive assessments, develop individualized care plans with families and the interdisciplinary team, and oversee care delivery with meticulous documentation.

Qualifications

Active RN license in Arizona (or state of practice) required.
Experience in Home Health or Hospice preferred.
Proven verbal and written communication skills and time management.

Responsibilities

Conduct comprehensive physical, social, and psychological assessments at patients' homes.
Collaborate with patients, families and Interdisciplinary Group to create care plans.
Manage care delivery and maintain accurate patient records.
Serve as a resource for families and facilitate referrals for holistic care.
