Job summary

Aegis Healthcare in Arizona is expanding its Home Health and Hospice Nursing Teams. The RN Case Manager role focuses on comprehensive home-based care and collaboration with patients and families to build personalized care plans.

You'll conduct assessments, coordinate interdisciplinary care, manage records, and support families with proactive referrals. Eligible candidates hold an AZ RN license and have Home Health or Hospice experience, with strong communication and organizational skills.

Qualifications

• Active RN license in AZ (or state of practice).
• Experience in Home Health or Hospice preferred.
• Strong verbal, written and time management skills.
• Valid driver's license with reliable transportation and proof of auto insurance.
• Ability to safely transfer patients.
• CPR certification and TB test (we can assist).
• AZ Fingerprint Clearance Card (we can help obtain).

Responsibilities

• Conduct comprehensive physical, social, and psychological assessments.
• Collaborate with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Manage care delivery and maintain meticulous patient records.
• Act as a resource for families, making proactive referrals, and fostering a holistic care experience.

Position Details

STATUS: Full-Time (mon-friday 8am-5pm w/ one 24 hour oncall shift per month)

COMPENSATION: $40-$45 /hour

LOCATION: Tucson and Oro Valley, AZ

Your Role As RN Case Manager

As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. Your role will include:

• Conducting comprehensive physical, social, and psychological assessments.
• Collaborating with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Managing care delivery and maintaining meticulous patient records.
• Acting as a resource for families, making proactive referrals, and fostering a holistic care experience.

Qualifications We're Looking For

• Active Registered Nurse (RN) license in Arizona (or state of practice).
• Previous Home Health or Hospice experience (preferred).
• Effective verbal, written, and time management skills.
• Valid driver's license, reliable transportation, and proof of current auto insurance.
• Ability to perform patient transfers with a full range of movement.
• Current CPR certification and negative TB test (we can assist).
• Valid AZ Fingerprint Clearance Card (we can help obtain it).
