Job summary

Aegis Healthcare is expanding our Home Health and Hospice Nursing Teams in Tucson, AZ. We’re seeking a dedicated RN Case Manager to assess, plan, and coordinate compassionate in-home care for patients, working with families and an interdisciplinary team.

Qualifications

• Active Registered Nurse (RN) license in Arizona
• Previous Home Health or Hospice experience (preferred)
• Effective verbal, written, and time management skills
• Valid driver's license, reliable transportation, and proof of current auto insurance
• Ability to perform patient transfers with a full range of movement
• Current CPR certification and negative TB test (we can assist)
• Valid AZ Fingerprint Clearance Card (we can help obtain it)

Responsibilities

• Conduct comprehensive physical, social, and psychological assessments.
• Collaborate with patients, families, and the Interdisciplinary Group to create individualized care plans.
• Manage care delivery and maintain meticulous patient records.
• Act as a resource for families, make proactive referrals, and foster a holistic care experience.

Position Details:

STATUS: Full-Time (mon-friday 8am-5pm w/ one 24 hour oncall shift per month)

COMPENSATION: $40-$45 /hour

LOCATION: Tucson and Oro Valley, AZ

Your Role as RN Case Manager:

As a Home Health & Hospice Nurse Case Manager, you'll be instrumental in providing exceptional care to patients in their homes. Your role will include:
