Job summary

AMN Healthcare seeks a qualified RN Case Manager in Salt Lake City, UT to coordinate patient care, discharge planning and care transitions with Epic and interdisciplinary teams.

RN Case Manager

StartDate: 11/2/2026 Pay Rate: $1800.00 - $2500.00

POSITION SUMMARY – RN Case Manager

Coordinates patient care, case management, and discharge planning by assessing patients’ medical, functional, and social needs; developing individualized care plans; collaborating with patients, families, physicians, and interdisciplinary teams; connecting patients with appropriate resources and services; monitoring care outcomes and resource utilization; advocating for patient needs; and supporting efficient, high-quality care throughout the continuum of treatment.

Minimum Required Qualifications

• MT/compact RN license
• BLS
• BSN
• 3 years of experience as a Registered Nurse in an acute care setting
• Hospital care management experience
• Knowledge of Medical Terminology
• Knowledge of Desk Top software including Power Point and Excel, computer skills for data entry

Preferred Qualifications

• 5 years nursing experience
• ACM or CCM certification
