Job summary

Bristol Hospice in Richmond, VA is seeking a compassionate RN Case Manager who will advocate for patients and families, delivering whole-person care in a setting that values dignity and comfort throughout the hospice journey.

You will develop individualized care plans, monitor vital signs, administer medications, educate families, and collaborate with an interdisciplinary team to ensure consistent, humane support.

Qualifications

• A heart for hospice care and a commitment to exceptional service.
• A valid Registered Nurse license in good standing in VA.
• A valid driver’s license and reliable transportation.
• CPR certification (current).
• Strong observational and communication skills—both written and verbal.

Responsibilities

• Develop and implement individualized care plans based on patient history.
• Deliver attentive, one-on-one support to patients and families.
• Assess physical, emotional, and spiritual needs throughout the care journey.
• Monitor and document vital signs with accuracy and timeliness.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.

QualificationsOn An Average Day You Will:

• Develop and implement individualized care plans based on current and prior medical history
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding
• Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure
• Administer physician-ordered medications and treatments with skill and empathy
• Educate patients and their families on care techniques and symptom management
• Track and communicate changes in patient condition, ensuring timely and effective responses
• Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned
