Job summary

Bristol Hospice Omaha, NE is seeking an RN Case Manager to provide compassionate, comprehensive hospice care and advocate for patients and families throughout the care journey.

In this role you will develop care plans, monitor patient status, educate families, and collaborate with an interdisciplinary team to deliver high-quality end-of-life care. Full-time, day shift with a strong mission-driven culture and opportunities for growth.

Qualifications

• Active RN license in good standing in the state you will work.
• Current CPR certification is required.
• Valid driver's license and reliable transportation.
• Excellent written and verbal communication skills.
• Ability to collaborate with interdisciplinary teams.

Responsibilities

• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs along the care journey.
• Monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer physician-ordered medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition with timely responses.
• Maintain up-to-date clinical notes reflecting patient progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

What You’ll Bring to the Team

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver’s license and reliable transportation
• Cpr certification (current)
• Strong observational and communication skills—both written and verbal
• A flexible, team‑oriented attitude and willingness to support additional duties as needed
