Job summary

Moments Hospice is seeking an experienced RN Case Manager to serve as the clinical lead for its patient population in a field-based capacity. You will manage symptoms, coordinate care, and support families during end-of-life transitions with a collaborative team approach.

The role offers a dedicated caseload and opportunities to shape clinical practice, with access to leadership and a supportive environment that values compassionate, patient-centered care.

Qualifications

• Active RN license in the state of employment.
• Proficiency with EMR systems.
• Strong organizational, communication, and critical-thinking skills.
• Ability to work independently while maintaining high standards of care.

Responsibilities

• Complete initial and ongoing comprehensive assessments of patients' needs.
• Develop, implement, and update individualized plan of care in partnership with patients and families.
• Provide skilled nursing care, including pain and symptom management, medication administration, and comfort measures.
• Educate patients and caregivers on disease progression, symptom management, medications, and what to expect at end of life.
• Coordinate care across your caseload and serve as the primary liaison between patients, families, physicians, and the care team.
• Participate in interdisciplinary group (IDG) meetings to ensure comprehensive, holistic care.

Pay: $75,000 – $90,000 per year

Job Type: Full-time | Field-based

Schedule: Monday–Friday, 8:30 AM – 5:00 PM

• Valid driver's license and reliable vehicle with current insurance
