This RN Case Manager position involves providing compassionate nursing care to hospice patients and families in Springfield and surrounding areas.

Responsibilities include assessing patient needs, coordinating with physicians and staff, maintaining accurate records, and educating patients and caregivers on medication administration, personal care, and safety.

The role requires prompt communication, documentation, and support during death and bereavement.

The RN participates in team meetings, evaluates aides and LPNs regularly, and maintains proficiency in pain management and hospice philosophy.

The schedule is Monday through Friday, 8am to 5pm, with after-hours admissions and death coverage as needed.

Prior experience in hospice, home health, or geriatrics is preferred.
