Insight Global Health is looking for a remote RN Case Managers to work for a large healthcare company in the Irvine, CA area. In this position, you will be working with outpatient members, providers, and caregivers to facilitate intensive, comprehensive care management. These RNs will complete Health Risk Assessments, care plans, and monthly follow-ups with members. They will also make post-discharge calls within transition of care and will be involved in interdisciplinary meetings.
The schedule is Monday to Friday, 8 AM to 5 PM. Candidate can sit anywhere in CA. Strong communication skills are a must and knowing some EMR and MS Tools is a plus.

- 2+ years of Case Management experience
- Unrestricted RN CA license
- Excellent computer skills
