Job summary

Transitions At Home is seeking a Full-Time Licensed Practical Nurse (LPN) to join our Madison area team. Enjoy a flexible schedule with 40 hours per week, 8:00 am–4:30 pm, Monday–Friday, no night shifts or weekends.

Qualifications

• Active Wisconsin LPN License required.
• Two years nursing experience; one year in Home Health preferred.
• CPR Certification required.

Responsibilities

• Provide skilled nursing care including routine visits, medication management and treatments.
• Coordinate services and counsel patient and family in their home as per physician orders and plan of care.
• Work under the direction of the RN Clinical Manager to ensure quality patient care.

What does a Transitions at Home LPN do?

Transitions At Home Licensed Practical/Vocational Nurse provide skilled nursing care to include, routine visits, medication management, treatments, and coordinates services, counsels the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care under the direction and supervision of the RN Clinical Manager.

Hours: Full-Time

• 40 hours per week 8:00 am - 4:30pm
• Monday through Friday
• No Night Shifts or Weekends
• Limited on-call rotation
