Job summary

Good Samaritan Hospice in Lynchburg, VA is seeking a compassionate Registered Nurse to join our team as an RN Case Manager. You will provide skilled nursing, assess needs, coordinate with the IDT, and deliver care with empathy during end-of-life journeys.

The position offers a Monday–Friday core schedule, competitive starting salary of $75,000 per year, PTO, benefits and 401(k) with company match. Virginia RN license and 2 years hospice/home health or acute care experience are preferred.

Qualifications

• Graduate of an accredited nursing program with an active Virginia RN license.
• Minimum of 2 years nursing experience (hospice/home health or acute care preferred).
• Comfort with EMR and technology.
• Reliable transportation with valid driver’s license and insurance.

Responsibilities

• Provide case management and coordinate care for assigned patients and families.
• Complete comprehensive assessments, admissions, and ongoing evaluations of patient needs.
• Collaborate with the Interdisciplinary Team (IDT) to develop and implement individualized Plans of Care.
• Deliver skilled nursing care including symptom management, IV therapy, wound care, and physical assessments.
• Educate patients and caregivers on medications, pain management, and daily care needs.
• Monitor patient condition and communicate changes to physicians and care team members.
• Maintain accurate, timely documentation in compliance with hospice policies and regulations.
• Provide emotional support and guidance to patients and families.
• Participate in IDT meetings, on-call rotation, and quality improvement activities.

What we are looking for:

Schedule & Compensation:

Schedule: Flexible hours around a core schedule (Monday-Friday approx. 8:30 AM–5:00 PM).

• $.65 per mile mileage reimbursement

Starting Salary: $75,000/year

This is the starting salary for the position. Compensation may vary within the established salary range based on relevant experience, education, licensure, certifications, and other qualifications.
