We are currently seeking compassionate, skilled Registered Nurse Case Managers to join our team. This is a combined job posting for two distinct opportunities within our organization: Hospice RN Case Manager and Home Health RN Case Manager. The Hospice Registered Nurse Case Manager participates as a member of the Interdisciplinary Team (IDT) to provide holistic care to patients and families. This role is responsible for comprehensive assessments, care coordination, and ongoing evaluation of patient and family needs. The RN ensures implementation and ongoing updates of the interdisciplinary plan of care to support comfort, dignity, and quality of life at end of life.
The Home Health RN Case Manager administers skilled nursing care to patients requiring intermittent professional services. This role focuses on coordinating, implementing, and evaluating care in the home setting in accordance with physician orders and individualized plans of care. The RN also provides patient and family education to promote independence, safety, and optimal health outcomes.
Schedule: Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on‑call rotation as required)
Location: Roanoke and surrounding service areas
