Job summary

Good Samaritan Hospice in Roanoke, VA is seeking a Home Health RN Case Manager to deliver patient-centered care at home or in hospice settings. You will coordinate, implement, and evaluate plans of care, conduct visits, and collaborate with an interdisciplinary team.

The role requires an accredited nursing degree, Virginia RN license, 2 years' nursing experience, and BLS. Starting salary is $75,000 per year, with comprehensive benefits and on-call rotation.

Qualifications

• Graduate of an accredited nursing program.
• Minimum of 2 years nursing experience (home health or hospice preferred).
• Active Virginia RN license (or compact license).
• Current BLS/CPR certification or obtained within 30 days of hire.
• Strong clinical assessment, communication, and decision-making skills.
• Comfort with EMR systems and mobile documentation tools.
• Valid driver’s license, reliable transportation, and proof of insurance.

• Develop, implement, and revise Plans of Care based on patient needs
• Complete OASIS documentation accurately and within required timeframes

Home Health RN Case Manager:

Job Summary

The Home Health RN Case Manager administers skilled nursing care to patients requiring intermittent professional services. This role focuses on coordinating, implementing, and evaluating care in the home setting in accordance with physician orders and individualized plans of care. The RN also provides patient and family education to promote independence, safety, and optimal health outcomes.

What you will do as a Home Health RN Case Manager:

• Conduct skilled nursing visits in patients’ homes

Schedule:

Monday–Friday, approximately 8:30 AM – 5:00 PM (with participation in on-call rotation as required)
