Adoration Health is seeking a dedicated Hospice RN Case Manager to join our Chattanooga team. This role focuses on patient-centered care, with responsibilities spanning assessment, care planning, and on-call support within a collaborative team.

The position emphasizes compassionate hospice care, accurate documentation, and caregiver education, with a Monday–Friday schedule and local coverage area in Hamilton, Marion, Sequatchie counties.

Qualifications

• Must have RN licensure to practice in the operating state.
• 1 year nursing experience required; 3+ years preferred.
• Experience in hospice or similar setting preferred.
• Knowledge of hospice philosophy of care.

Responsibilities

• Assess and monitor physical, emotional, and psychological needs of patients.
• Create hospice care plans aligning with patient wishes and goals.
• Direct nursing care: administering medications, treatments, and interventions.
• Provide pain and symptom management.
• Educate and support the patient’s family and caregivers.
• Collaborate with interdisciplinary team.
• Maintain accurate and timely documentation.
• Participate in on-call rotation as required by local branch.

Adoration Hospice is seeking a passionate, dedicated Hospice RN to join our team in Chattanooga, TN. Our hospice RNs provide expert, patient-centered care during a critical time in people’s lives. The position is Monday‑through‑Friday, 8am‑5pm. Office Location: Chattanooga, TN. Coverage area: Hamilton, Marion, Sequatchie counties.

As a RN Case Manager You Will

Qualifications

• Graduate of an accredited nursing school with current licensure to practice in the state of operation
• One year nursing experience required, 3+ years preferred

• Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
