Prime Healthcare is seeking a per diem RN at Harlingen Medical Center in Harlingen, TX. Shifts are Days (weekends) with 8-hour blocks, focusing on patient assessment, care coordination, discharge planning, and utilization review.

The role requires a BSW or related bachelor’s degree, RN license, and 1+ year in case management or discharge planning. Preferred CCM and BCLS; Milliman/InterQual knowledge is a plus. Benefits include medical insurance and a 401K.

Formación

• BSW or other bachelor's degree in a related field. At least one year experience in case management, discharge planning or nursing management; or2. Experience and knowledge in basic to intermediate computer skills.
• RN License is required.
• Preferred CCM certification or obtain within 1 year and Basic Life Support (BCLS) certificate preferred.

Responsabilidades

• Evaluate patients’ medical and discharge needs upon admission and during stay.
• Develop and implement individualized care plans with the healthcare team.
• Coordinate safe discharge plans including referrals to rehab, home health, or long-term care facilities.
• Monitor hospital resource use to ensure appropriate care and avoid unnecessary stays.
• Educate patients and families about care plans, post-discharge instructions, and resources.
• Advocate for patient needs and preferences to align care with goals.

Location: Harlingen Medical Center - Harlingen, TX

Responsibilities

• Patient Assessment: evaluate patients’ medical, psychosocial, and discharge needs upon admission and throughout their stay


• Care Coordination: develop and implement individualized care plans in collaboration with the healthcare team to ensure effective treatment and timely interventions


• Discharge Planning: coordinate safe and appropriate discharge plans, including referrals to rehab, home health, or long-term care facilities


• Utilization Review: monitor the use of hospital resources and services to ensure appropriate care and avoid unnecessary treatments or extended stays


• Patient and Family Education: help patients and families understand care plans, post-discharge instructions, and available resources


• Advocacy: advocate for patient needs and preferences, ensuring care is aligned with their goals and values

Qualifications

1. BSW or other bachelor's degree in a related field. At least one year experience in case management, discharge planning or nursing management; or2. Experience and knowledge in basic to intermediate computer skills.

3. RN License

Preferred qualifications:

1. Minimum 5 years of acute care experience preferred.2. CCM or obtained within 1year3. Current BCLS certificate preferred.4. Knowledge of Milliman Criteria and InterQual Criteria preferred.
