Job summary

Harlingen Medical Center is seeking a Part Time clinical professional to support case management, discharge planning and patient advocacy.

The role requires a BSW or related degree with RN license and at least one year of relevant experience. Strong computer skills and knowledge of Milliman and InterQual criteria are preferred.

Qualifications

  • BSW or related bachelor’s degree required; At least one year in case management, discharge planning or nursing management.
  • RN license required.
  • Basic to intermediate computer skills required.

Responsibilities

  • Assess patients upon admission and during stay to evaluate medical, psychosocial, and discharge needs.
  • Coordinate and implement individualized care plans with the healthcare team.
  • Plan safe and appropriate discharge, including referrals to rehab or home health.
  • Monitor hospital resources to ensure appropriate care and avoid unnecessary treatments or extended stays.
  • Educate patients and families on care plans, post-discharge instructions, and available resources.
  • Advocate for patient needs and preferences to align care with goals and values.

Employment Type: Part Time

Hours: 8 hour shifts

Location: Harlingen Medical Center - Harlingen, TX

Responsibilities
  • Patient Assessment: evaluate patients’ medical, psychosocial, and discharge needs upon admission and throughout their stay
  • Care Coordination: develop and implement individualized care plans in collaboration with the healthcare team to ensure effective treatment and timely interventions
  • Discharge Planning: coordinate safe and appropriate discharge plans, including referrals to rehab, home health, or long-term care facilities
  • Utilization Review: monitor the use of hospital resources and services to ensure appropriate care and avoid unnecessary treatments or extended stays
  • Patient and Family Education: help patients and families understand care plans, post-discharge instructions, and available resources
  • Advocacy: advocate for patient needs and preferences, ensuring care is aligned with their goals and values
Qualifications
  • BSW or other bachelor's degree in a related field. At least one year experience in case management, discharge planning or nursing management; or
  • Experience and knowledge in basic to intermediate computer skills.
  • RN License
Preferred Qualifications
  • Minimum 5 years of acute care experience preferred.
  • CCM or obtained within 1year
  • Current BCLS certificate preferred.
  • Knowledge of Milliman Criteria and InterQual Criteria preferred.
