Job summary

Caring Hospice Services is seeking an RN Case Manager to join our on-the-road team providing compassionate, patient-centered hospice care. You will coordinate interdisciplinary plans of care, connect patients and families with resources, and support caregivers across Massachusetts.

Active MA RN license required; BSN preferred. Hospice experience is highly valued. A valid MA driver’s license and auto insurance are required to visit patients at home and in facilities throughout the region.

Qualifications

• Active Massachusetts RN license in good standing.
• Bachelor's degree in nursing preferred.
• Hospice experience preferred.
• Strong nursing skills and the ability to build supportive connections with patients and families.
• A valid driver’s license and auto insurance in Massachusetts.

Responsibilities

• Coordinate care as an integral member of the interdisciplinary team.
• Collaborate with hospice colleagues, volunteers, physicians and community partners to implement plans of care.
• Develop and implement individualized plans of care focused on comfort and quality of life.

As an RN Case Manager, you will be an integral member of the interdisciplinary team, coordinating care and connecting patients, families, and caregivers with the resources they need throughout their hospice journey. You will collaborate with hospice colleagues, volunteers, physicians, and community partners to implement individualized plans of care focused on comfort and quality of life.

What You Bring

• An active Massachusetts Registered Nurse license in good standing
• Bachelor’s degree in nursing is preferred
• Hospice experience preferredStrong nursing skills and the ability to build supportive connections with terminally ill patients and their families
• The ability to assess, adapt, and respond to patient needs across a variety of care settings
• A valid driver’s license and auto insurance in Massachusetts
