Brookhaven Hospice seeks a Compassionate Registered Nurse Case Manager (RNCM) to join our dedicated team in Southbridge area. This full-time role involves hands-on hospice nursing, care coordination, and guiding patients and families through the hospice journey.

The position offers a Monday–Friday schedule (8:00 am–4:30 pm) with a $95,000–$105,000 annual pay range and a $15,000 sign-on bonus. A supportive benefits package and ongoing onboarding are provided to promote growth and patient-centered

Qualifications

• Current unencumbered RN license.
• Minimum 1 year RN experience in medical-surgical/acute care; hospice exp a plus.
• Proficient in computer use and EMR.
• CPR certification (if required by state).
• Valid driver's license and reliable transportation.

Brookhaven Hospice is seeking a Full Time Hospice RN Case Manager (RNCM) to join our dedicated nursing team providing exceptional end-of-life care throughout the community.

Schedule: Monday - Friday, 8 am - 4:30 pm

Territory: Southbridge, Sturbridge, Charlton, Dudley and Brookfields

• Collaborative Care Champion: Partner closely with our Clinical Director to assess the eligibility and suitability of clients for our specialized hospice services. Your expertise will guide us in providing the right care, at the right time.

Qualifications

• Hold current unencumbered license as a Registered Nurse.
• Minimum one (1) year experience as an RN in a medical surgical/acute care setting. Hospice exp a plus!
• Must be computer proficient in typing and various programs, including background in EMR.
• Possess and maintains current CPR certification if required by state.
• Current driver's license, vehicle insurance, and access to dependable transportation.

$95,000 - $105,000 per year (Average Pay Range). The pay range listed represents a general posting guideline for the role and is not a fixed offer. Final compensation will be determined based on the candidate’s relevant experience, qualifications, and the specific responsibilities of the position. The exact compensation rate will be discussed and confirmed at the conclusion of the interview process.
