Job summary

Bristol Hospice is seeking an RN Case Manager to support patients and families across Des Plaines, Elk Grove, Schaumburg, Hoffman Estates, Bloomingdale and Itasca. You will develop care plans, monitor symptoms, educate families, and coordinate with interdisciplinary teams to ensure dignity and comfort at end of life.

Qualifications

  • Heart for hospice care and commitment to exceptional service.
  • Valid Registered Nurse license in the state of employment.
  • Valid driver's license and reliable transportation.
  • Current CPR certification.
  • Strong observational and written/verbal communication skills.
  • Team-oriented stance with willingness to take on additional duties.

Responsibilities

  • Make a difference for patients and families with compassionate care.
  • Develop and implement individualized care plans based on medical history.
  • Provide one-on-one support ensuring comfort, dignity, and understanding.
  • Conduct thorough assessments of physical, emotional, and spiritual needs.
  • Monitor and document vital signs accurately.
  • Administer physician-ordered medications and treatments with empathy.
  • Educate patients and families on care techniques and symptom management.
  • Track changes in patient condition and respond promptly.
  • Maintain detailed clinical notes reflecting progress and quality of care.
  • Collaborate with interdisciplinary teams for comprehensive care.
  • Contribute to other duties as part of a mission-driven team.
  • Perform additional duties as assigned.
