Job summary

Bristol Hospice in North Dallas/Addison, TX is seeking a dedicated RN Case Manager to provide compassionate, patient- and family-centered care. This full-time role offers a competitive salary and benefits within a supportive, mission-driven team environment.

Qualifications

• Must hold a valid RN license in good standing in the state of employment.
• CPR certification (current).
• Excellent observational and written/verbal communication skills.
• Flexible, team-oriented attitude; willing to perform supportive duties.
• Reliable transportation and driver's license.

Responsibilities

• Make a true difference - one patient, one family, one compassionate moment at a time.
• Develop and implement individualized care plans based on current and prior medical history.
• Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding.
• Conduct thorough assessments of physical, emotional, and spiritual needs.
• Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure.
• Administer medications and treatments with skill and empathy.
• Educate patients and families on care techniques and symptom management.
• Track and communicate changes in patient condition, timely responses.
• Keep detailed clinical notes reflecting progress and quality of care.
• Collaborate with interdisciplinary teams to ensure comprehensive care delivery.

• Collaborate with interdisciplinary teams to ensure comprehensive care delivery
• And contribute to other supportive duties as part of a compassionate, mission-driven team
• And additional duties as assigned

What You'll Bring to The Team:

• A heart for hospice care and a commitment to exceptional service
• A valid Registered Nurse license in good standing in the state you will work in
• A valid driver's license and reliable transportation
• CPR certification (current)
• Strong observational and communication skills - both written and verbal
• A flexible, team-oriented attitude and willingness to support additional duties as needed
