Job summary

Cynet Health is seeking an RN Case Manager in Charleston, SC for a 13-week traveler assignment. The role focuses on coordinating comprehensive case management services in an acute care hospital setting.

The ideal candidate has at least 2 years of case management experience, an active RN license, and familiarity with hospital documentation systems. Shifts run 8:00 AM–4:30 PM with on-call/weekend rotation as required.

Qualifications

• Must have at least 2 years of clinical experience in case management.
• Licensed Registered Nurse in good standing.
• CCM or ACM certification preferred but not required.

Responsibilities

• Coordinate patient care across hospital units and services.
• Manage caseload to meet facility policies and patient needs.
• Schedule and rotate weekends/holidays per policy.
• Document patient interactions in MediTech, Midas, CarePort, Kronos.
• Adhere to hospital policies and guidelines.

Description:Traveler shall be scheduled in accordance with facility policy.The traveler is expected to work every other weekend and holidays in rotation with hospital employees.1+ years of experience is acceptable.The unit has 321 beds and operates as an acute care hospital.Patient ratios in the unit range from 1:25 to 1:30.There are 2 Case Management Assistants in the department.Documentation systems utilized include MediTech, Midas, CarePort, Facility Scheduler, and Kronos.Scheduling requirements include Monday to Friday from 8:00 AM to 4:30 PM with weekend rotation as needed.Floating policy is applicable within hospital units.The ideal candidate should have acute hospital case management experience.Travel experience is preferred.Business casual attire is required.Supportive management and an emphasis on teamwork are core tenets of the staff environment.The work-life balance is highly regarded by team members.The location is rich in cultural history and offers various recreational activities.This environment provides an opportunity to experience Southern charm and beautiful coastal activities.
