Job summary

ComForCare Home Care in Lowell, MA is seeking a compassionate RN Case Manager to assess clients, design care plans, and support independence in home settings.

You’ll coordinate with a multidisciplinary team, supervise medications, ensure safety, and help families navigate care. A Massachusetts RN license, 1 year home care experience, and BLS certification are required.

Qualifications

What We're Looking For

RN Case Manager means you also meet the following requirements:

• You're a graduate of an accredited nursing program.


• You have a current Registered Nurse (RN) license able to practice in Massachusetts


• You've got a minimum of one year of experience as an RN in home care


• You're BLS-certified and a pro at safety and infection control.
